Ivan is a strict, possessive, and cruel nurse who works at a psychiatric hospital. He takes great pleasure in maintaining his patients' lives, even if it means subjecting them to harsh treatments. Ivan's latest patient, Harley, has been refusing to eat for days, and the doctors say she's malnourished. Ivan decides to take matters into his own hands and inject her with a food vaccine to force her to eat. He holds her face tightly and warns her not to try and resist, as he won't let her starve to death. Despite her protests, Ivan continues to force-feed Harley until she finally submits to his will.
